Prescription onlyEnclomiphene

Mechanism

Signal up, don't substitute.

Enclomiphene blocks estrogen receptors in the pituitary, which increases LH and FSH output and tells the testes to produce more testosterone themselves.

Because production is stimulated rather than replaced, testicular function and sperm production are preserved, the opposite of what conventional TRT does.

It is a tablet rather than an injection, and it is not FDA-approved for this use.

Enclomiphene

Evidence

What the research shows.

Enclomiphene raises endogenous testosterone while maintaining sperm concentration, which conventional testosterone therapy does not.

It is not FDA-approved for this indication and is prescribed off-label.

Results vary. Clinical trial results apply only to the FDA-approved branded medication specifically identified and do not apply to compounded medications. All medications must be prescribed by a licensed provider based on medical necessity.

Safety and eligibility

Your consult and labs screen for all of the following.

  • Not appropriate for primary testicular failure, it requires a responsive pituitary–testicular axis.
  • Tell your provider about any history of blood clots or visual disturbance.
  • Common side effects include headache, nausea and mood change.
  • Not FDA-approved for this use, prescribed at your provider's discretion.

FAQ

Enclomiphene questions.

For men who want to preserve fertility, generally yes, it raises your own production rather than replacing it. For men with primary testicular failure it will not work, because it depends on the testes being able to respond.

It preserves it. Unlike testosterone therapy, which suppresses sperm production, enclomiphene stimulates the natural signalling pathway and typically maintains or improves sperm concentration.

Testosterone levels usually begin rising within two to four weeks, with symptom improvement following. Repeat labs at six to eight weeks confirm the response.
