Cognitive Support
Methylene Blue
A low-dose mitochondrial agent used off-label for focus, mental clarity and cellular energy.
This is a compounded preparation made by a licensed pharmacy. It is not FDA-approved, and it is prescribed only when a licensed provider determines it is appropriate for you.
Already know this is what you want? The second option books the appointment straight away. Not sure yet? See the labs first.
Your consult is free. We will not prescribe this, or anything, before a clinician has spoken with you. Final cost depends on the form and dose prescribed.

Mechanism
An alternative electron carrier.
At low doses methylene blue acts as an alternative electron carrier in the mitochondrial transport chain, supporting ATP production when normal pathways are impaired.
It also has antioxidant activity at these doses. The dose-response is biphasic, low doses behave very differently from high ones, which is why self-dosing is a bad idea.
Only pharmaceutical-grade material is appropriate. Industrial or aquarium-grade methylene blue is not a medication and can contain heavy-metal contaminants.

Protocol
How treatment usually runs.
A typical schedule. Yours is set by your clinician, and adjusted as you go.
| Phase | Weeks | Typical | Purpose |
|---|---|---|---|
| Start | 1–2 | Low dose, once daily | Blue-green urine is expected and harmless. |
| Maintenance | 3+ | Adjusted to response | Kept in the low-dose range. |
What to expect
Honestly, over time.
Blue urine
Expected, harmless and slightly alarming the first time.
Focus
Where responders typically notice a change.
Steady
Effects are subtle and cumulative rather than acute.
Reassess
Stopped if no benefit is apparent.

Evidence
What the research shows.
Methylene blue has demonstrated mitochondrial and cognitive effects in preclinical work and small human studies.
Evidence for routine cognitive enhancement in healthy adults is limited. Prescribed off-label.
Results vary. Clinical trial results apply only to the FDA-approved branded medication specifically identified and do not apply to compounded medications. All medications must be prescribed by a licensed provider based on medical necessity.
Safety and eligibility
Your consult and labs screen for all of the following.
- Do not combine with SSRIs, SNRIs, MAOIs or other serotonergic medication, serotonin syndrome risk is real and serious.
- Do not use with G6PD deficiency.
- Avoid in pregnancy and breastfeeding.
- Expect blue-green urine. Only pharmaceutical-grade material is appropriate.
FAQ
Methylene Blue questions.
No. Methylene blue is a monoamine oxidase inhibitor and combining it with SSRIs, SNRIs or MAOIs risks serotonin syndrome, which can be life-threatening. Tell your provider every medication and supplement you take.
It is excreted renally and retains its color. Blue or blue-green urine is expected, harmless, and typically appears within hours of the first dose.
Low doses, well below those used in emergency medicine. The dose-response is biphasic, higher doses have the opposite effect on mitochondria, which is why this requires a provider and pharmaceutical-grade material.
