Mechanism
Four actives targeting pigment.
Hydroquinone and kojic acid both inhibit tyrosinase, the enzyme that drives melanin production, which is why they are paired here, each targets the same pathway from a slightly different angle.
Azelaic acid adds a second, gentler depigmenting and anti-inflammatory pathway, useful for the redness that can accompany melasma, and low-dose hydrocortisone is included to reduce irritation from the other actives.
Hydroquinone is one of the most effective treatments for stubborn pigmentation, but it is intended for limited courses under clinician supervision, prolonged unsupervised use carries a risk of a separate pigment condition called ochronosis, which is why this is prescription-only and monitored.
